Informed consent is a fundamental ethical and legal requirement in healthcare, ensuring that patients understand the nature, risks, and benefits of medical interventions before consenting to treatment. In emergency care, the challenge of obtaining informed consent becomes even more complex due to the urgent nature of medical conditions and the often limited time available for patient-provider communication. Lagos University Teaching Hospital (LUTH) is one of Nigeria’s largest and most reputable healthcare institutions, providing emergency care to a vast urban population. The hospital’s emergency department is consistently busy, managing both life-threatening and less acute cases. Given the diverse patient population, obtaining informed consent in a way that ensures patients understand their treatment options, even in emergency settings, is crucial.
In emergency settings, the urgency of care sometimes necessitates immediate interventions, and patients may be unable to provide consent due to their medical condition (e.g., unconsciousness, mental incapacitation). In such cases, healthcare providers rely on implied consent or the authority of surrogate decision-makers, which raises ethical and legal concerns. Furthermore, cultural, language, and literacy barriers may complicate the informed consent process, especially in a multiethnic and multilingual population like that of Lagos.
This study will examine the current practices of obtaining informed consent in emergency care at LUTH, with a focus on how nurses and other healthcare providers approach consent in emergency situations. By exploring the factors that influence informed consent in emergency care, the study aims to identify potential improvements in the process, ensuring that patients’ rights are upheld and that care is delivered in a manner consistent with ethical and legal standards.
Statement of the Problem (300 words)
Informed consent is a cornerstone of ethical healthcare practice, but its application in emergency care is often complicated by factors such as urgency, patient incapacity, and time constraints. At Lagos University Teaching Hospital (LUTH), where emergency services cater to a large, diverse population, obtaining informed consent presents significant challenges. In critical situations, patients may not be able to fully understand or participate in the consent process due to the nature of their condition or their inability to communicate effectively.
Moreover, healthcare providers, including nurses, may face difficulties in ensuring that patients are adequately informed, especially in emergency settings where decisions must be made rapidly. The hospital's emergency department often sees patients in varying degrees of distress, and ensuring that consent is given voluntarily and with full understanding can be challenging under such circumstances. Additionally, language barriers, low health literacy, and cultural differences may further complicate the process.
This study aims to examine how informed consent is currently practiced in emergency care at LUTH, evaluating the procedures followed, the barriers faced, and the effectiveness of these practices in ensuring patient autonomy and informed decision-making. The findings will provide insights into areas where improvements are needed, including the development of clearer protocols and strategies for overcoming challenges in obtaining informed consent in emergency care.

1. H₀: There are no significant challenges in obtaining informed consent in emergency care at Lagos University Teaching Hospital.
2. H₀: Healthcare providers' practices do not significantly affect the quality of informed consent obtained in emergency settings.
3. H₀: There are no effective strategies to improve the informed consent process in emergency care at Lagos University Teaching Hospital.
Scope and Limitations of the Study
This study will focus on informed consent practices in emergency care at Lagos University Teaching Hospital. Limitations may include variations in practice across different healthcare providers, difficulty in assessing patient understanding, and language barriers in a diverse patient population.
